After scheduling your appointment, please complete your New Patient Paperwork/ Health Questionnaire before your appointment time. This helps us to be as efficient as possible and saves you time in the office.
Fill out the New Patient Paperwork online and have it securely sent to our office, or print off and complete one of the forms below (Paediatric form for children under 12). Plan for approximately 45 minutes on your first visit to allow enough time for a thorough consultation and examination.
What to Expect on Your First Visit
When you walk into Vida Chiropractic Maria or Mani will greet you at the front desk and double check that all your paperwork is complete. Help yourself to a glass of water and have a seat for a moment until a chiropractic assistant guides you back to an exam room.
At Vida Chiropractic we focus on education so you understand everything we’re doing and how it will help you with your condition. Feel free to ask any questions as we go along!
The next step is to meet with the doctor to have your assessment completed. The doctor will perform a comprehensive exam of your spine, looking for areas of subluxation and abnormal posture. The exam will include range of motion testing, postural analysis, orthopedic/neurological tests and digital thermography or static sEMG scans of your spine using Myovision technology.
Based on the findings from the exam the doctor may request chiropractic postural x-rays of your spine. These help provide information such as the position of vertebrae and the health of your spine. If x-rays are necessary, any additional costs will be discussed prior to them being taken.
***If you have already had x-rays taken, or any other diagnostic tests (CT scan, MRI, etc) please bring them with you on your first visit if possible.
What to Expect on Your Second Visit
Your second visit is one of the most important visits you will have in our office. At this visit we will go over your findings from your assessment and x-rays (if taken) as well as give our recommendations for you to acheive the best and fastest results possible. After this visit you will a better understanding of your condition and how chiropractic can help with your recovery. This visit may take up to 90 minutes so we ask that you budget your time accordingly.
